Domestic Partner
On November 21, 2002, the AWC Employee Benefit Trust Board of Trustees approved domestic partners as an eligible dependent for enrollment in all benefit programs (medical, dental, and vision). Enrollment of domestic partners and their eligible children remains a jurisdictional decision.
The Board feels that adoption of a policy that extends Trust benefits to domestic partners is a good business decision. However, the Board is clear in its direction that the ultimate decision to extend coverage is the sole responsibility of each member’s governing body.

Required City Documentation
The following documents will need to be provided to the AWC Trust:
- The AWC will need to receive a copy of the employer’s city-wide personnel policy addressing domestic partner eligibility criteria.
- Letter of intent to provide Domestic Partner coverage, including effective date.
- Updated AWC Employer Master Participation Agreement (pdf, 101 kb)
- Affidavit of Marriage/Domestic Partnership Form (Employees fill out)
- AWC Combined Enrollment Form (Domestic Partner Version) (Employees fill out)
