Is your mini-grant waiting for you?

Would you like to plant an employee garden? Furnish a private break room for employees to take stress breaks? Maybe hire a great motivational speaker, buy a new treadmill, get some kitchen tools to encourage healthy eating, or hire a yoga instructor for some onsite classes? These are the kinds of things you can do with your mini-grant!

Mini-Grants are intended to fund follow-up education and activities that target high risk areas identified by the annual Health Questionnaire. Each Trust member who reaches 50% overall participation in the HQ qualifies for a mini-grant.

Grants are normally worth $5 per employee (with a minimum of $50 and maximum of $500), but if you reach the 50% mark by September 1 the Trust doubles the amount of your grant! In 2014, 146 Trust members qualified for grants ranging from $50 – $1,000!

If you qualified for a grant in 2014, you have until July 1 to use it. To use your mini-grant:

  1. Check the list to confirm the amount of your grant.
  2. Select a health risk to target with your mini-grant activity.
  3. Fill out the brief mini grant application to describe how you plan to spend your grant. Contact Julie, Niki, or Jon for activity ideas.
  4. Email your completed proposal form to Jon, or mail to Jon Smith, AWC, 1076 Franklin Street SE, Olympia, WA 98501.
  5. After your proposal is approved by AWC, plan and conduct activities.
  6. Submit a mini-grant request for reimbursement form along with your receipts. AWC will reimburse receipts up to your eligible amount.
  7. Activities must be completed, and receipts submitted, by July 1, 2015.
